This form gives us important contact and emergency information.  We will use this information to communicate with you via telephone, mail or e-mail.  We also use this information to complete claim forms in the event that your child requires a referral to a doctor or specialist.  It is important that all information is correct and current, especially phone numbers.

LISD Physicals, Insurance & Medical Procedures
Medical Information Release

This form explains information concerning physicals, insurance and medical procedures.  All students entering 7th, 9th and 11th grades require a physical.  Other students may need a new physical as indicated on the medical history form.  LISD provides a secondary accident insurance policy to help supplement payments from parent's insurance policies.  This insurance coverage is not a zero balance policy and may not cover all expenses.  By signing this form, you acknowledge that you agree to the policies and procedures explained in the form.  You also give permission to coaches and athletic trainers to authorize a doctor to examine and treat your child.

Medical information that we regularly share includes, but is not limited to, injury reports to coaches, physicians and LISD athletic trainers; personal and medical information to insurance companies regarding insurance claims; other school districts in the event that the student should transfer to another district.  By signing this form, you authorize the release of confidential health information in the form of medical records or a summary or narrative of your protected health information to the persons or entities listed on the form.  If you would prefer to limit the information or persons or entities, please list the limitations on the lines provided.

UIL Medical History

This form must be completed by the parent/guardian and student in order for the student to participate in athletic activates.  These questions are designed to determine if the student has developed any condition which would make it hazardous to participate in an athletic event.  Please explain and "yes" answers in the box at the bottom right of the page.  Yes answers to questions 1-6 require further evaluation which may include a physical examination.  By signing this form, you acknowledge that you have answered all questions truthfully and completely.  You also give consent for physicians, athletic trainers, nurses or school representatives to treat your student.

UIL Pre-Participation Physical Evaluation
*Completed Biannually or as indicated on medial history form*
*Physical Signature Required*

This form must be completed prior to junior high athletic participation and again prior to first and third years of high school. It must be completed and on record with the high school athletic trainer or junior high school coach before the student participates in any practice, before, during or after school, (both in-season and out-of-season) or games, scrimmages or matches.

UIL Acknowledgement of Rules Page 1 of 2

This form gives consent to compete in UIL approved sports, and travel with the coach or other school representative on any trips.  By signing this form, you acknowledge that even through protective equipment is worn by the athlete whenever needed, the possibility of an accident still remains and neither the UIL nor LISD assumes any responsibility in case an accident occurs; you agree to be responsible for the safe return of all athletic equipment issued by LHS to your child; you have read and understand the UIL rules on page 2 of this form (shown below) and agree that your child will abide by all of the UIL rules.

This form gives general information and explains general eligibility rules for UIL competition.  By signing this form, the student agrees to follow the rules explained; you understand that failure to proved accurate and truthful information on UIL forms could subject you to penalties determined by the UIL.

LISD Drug Testing Policy Acknowledgement

LISD's drug testing policy applies to middle and high school students who plan to participate in athletics, band, choir, cheerleading or Viewettes/Drill Team.  Students are not allowed to participate until this form is signed, dated and returned to their coach, advisor or athletic trainer.  By signing this form, you acknowledge that the student is to follow the extracurricular activity policies of LISD and give consent to participate in LISD's drug testing program.  A copy of LISD's drug testing policy can be viewed by clicking this link.

UIL Steroid Use & Testing Acknowledgement Agreement

*Parent/Guardian & Student Signatures Required*

Texas state law prohibits possessing, dispensing, delivering or administer a steroid in a manner not allowed by state law.  Any violation of state law concerning steroids is a criminal offense punishable by confinement in jail or imprisonment in the Texas Department of Criminal Justice.  By signing this form, the parent/guardian and student agree that he or she will not use anabolic steroids and acknowledge that he or she may be asked to submit to testing for the presence of anabolic steroids and submit to such testing and analysis.
