SAT Tactical Strategies Course Registration Form

Use this form to
register to take the SAT prep course. It is recommended that you take the
course session that corresponds to the date on which you intend to take
the SAT test. Each course is comprised of 3 separate sessions and all 3
sessions are included in the $45 fee. The fee is not pro-rated; if you are
not able to attend all three sessions, you will still be required to pay

Student Data Sheet - Use this form to assist the people you choose to write letters of recommendation for you. Complete the form, make several copies, and give one to each letter writer. The information that you provide on the form will help teachers, counselors, employers, pastors, etc. compose an excellent recommendation for you. Be sure and be thorough and explanatory on your form. Even though YOU know may know what "MYA" stands for your recommender may not. Remember he/she will disregard the items that they don't understand. Scholarship Offer Reporting Form - Use this form to list the scholarships that you have been offered by a school, an organization or individual. Please use the complete and official name of the scholarship and the organization that offers you a scholarship. Even though the form asks for the value (amount) of the scholarship, please know that we will never publish the amount of the scholarship. We use the value to calculate the total amount of awards given to LHS students each year. You can turn in as many of these as you need to Mrs. Milam. Graduation Plan Worksheet - This form is for parents and students to use to track progress toward graduation. If you are currently a 9th or 10th grader, please see the footnote at the bottom of the form as it relates to changes in science graduation requirements. Current 11th and 12th graders are required to have a minimum of 3 credits of science to graduate. Current 9th and 10th graders must complete 4 credits. For detailed information on state requirements for graduation, please read the information in the LHS Course Selection Guide, beginning on page 6. 2008 - 2009 Course Selection Sheet - This form can be used by students and parents as a "worksheet" to assist them in choosing courses for the next school year. It is NOT the official form that should be submitted to the student's counselor. Students will be issued a special form to submit. |
