A note to students and parents...

NCAA Clearinghouse no longer accepts paper student release forms that you can mail in. Students must complete the release form online. The statement that appears on the website is as follows:
 

An Adobe Acrobat® PDF version of the Student Release Form is no longer available. All registrations must now be completed online.

Follow these steps to complete the student release form online:

  1. Click here to go to the NCAA Clearinghouse Student Release Form page.

  2. Read the top portions carefully and then scroll down to the online form.

  3. Complete the form and then click "Continue."

  4. Proceed through the form and follow the instructions.
