Birth Certificate

An official copy of the record of birth is required at the time of registration of new students. A school certificate, obtainable without charge from the Texas Department of Health, will be accepted.

Immunization Requirements

Diphtheria, Tetanus, Pertussis containing vaccine (DPT/DTaP/DT) Children entering Kindergarten will be required to show five doses of DTP/DTaP/DT. Five doses are required unless the fourth dose was administered on or after the 4th birthday.
Students 7 years or older:
• Three doses of any combination DPT/DTaP/DT/Td (pertussis vaccine is not required)
• One dose of Td required ten years after the last dose of DTP/DTaP/DT

Polio (IPV) Children entering Kindergarten will be required to show proof of four doses of polio vaccine, unless the 3rd dose was on or after the 4th birthday.

Measles, Mumps, Rubella (MMR) two doses of a measles-containing vaccine with the first dose on or after the first birthday; the second dose by age 5 or entry into kindergarten.

Hepatitis B Students will need to show proof of the hepatitis B vaccination by grade level. Kindergarten through 11th grade will be required to show proof of three doses of hepatitis B vaccine.

Varicella (Chickenpox)* One dose on or after the first birthday for the following grades:
K-5 and 7-11 for the 2005-06 school years
K-12 for the 2006-2007 school years
(Two doses required if vaccine is given at 13 years or older)
* Documented proof of the disease is accepted

All Immunizations should be completed by the first day of attendance. The law requires that students be fully vaccinated against the specified diseases.
 

Medication at School

All medication at school must be administered by the school nurse. This includes any over the counter medication, such as aspirin. Students must have completed and signed by parent/guardian a consent form which is available in the nurses office. This sheet was distributed to all students at the beginning of the school year.
